SOFTBALL ONTARIO INCIDENT REPORT 

What is the purpose of this report?

The purpose of the Softball Ontario Incident Report is to report any form of abuse of a Softball Ontario official.  This form takes you through the proper steps in order to document the incident correctly.

If a local league feels that this form could be useful at the local level then please feel free to use it.  If reporting things such as player injuries, please note that Softball Ontario does not deal with such  items, and there is no need to report these to us. This might be something that you would want to take to your local league or Provincial Association.

Who Do I Send this Form to?

If a situation happens at a local tournament or league game be sure to send a copy of this form to that Local League, as well as Softball Ontario. Softball Ontario will keep this report on file as well as sending it to the Umpire Committee.

It is important that you keep a copy of this report for yourself.
